Transcript for: How to Self-Enroll in My Connected Care

My Connected Care allows you to access your health records from Grand River Hospital and St. Mary's General Hospital. To enroll yourself in my connected care you'll need the following information: your first name and last name as it appears on your health card, your date of birth and either of the following: Your health card number or medical record number at either hospital OR your email address as it's listed in our system. You can find your medical record number on an arm band or documents given to you at the hospital. If you don't have this information but would like to enroll, fill in our self-enrollment assistance form on our website. A member of our team will verify your information and will send you an email with instructions within a few business days.

If you have the information you need. Click on the self-enrollment link on our website. The first step is to fill in the self-enrollment form. Enter your first name and last name as they appear on your health card. Then enter your date of birth. Drop down to select the month and entering four digits for the year. To enroll with personal identifiers, enter your health card number with no letters, dashes or spaces. Then enter the last four digits of your medical record number. Then select, I'm not a robot and complete the puzzle. Confirm your identity and agree to our terms of use. To enroll with an email address already listed at the hospital enter your email address and complete the puzzle. Keep the window open. You will receive a verification code that is valid for 30 minutes. Enter the code and press continue. If you don't receive this email check your spam or junk folder. The last step is to set up your account. First, enter your email address. then enter a strong password using at least three of the criteria. Enter your name and agree again to the terms of use and age restriction. Once you set up your my connected care account you'll be able to see sections of your health record, read messages from your care team, and see upcoming appointments.
