My Connected Care is a free, secure online tool that gives you access to your personal health information. This online service is used across:
Grand River Hospital
St. Mary’s General Hospital, and
Grand River Regional Cancer Centre
You can use My Connected Care to view test results, clinical documents, and other health information using your computer or personal device (e.g. cell phone, tablet). With My Connected Care, you can view most of your health information close to the time it is posted in our hospital system (with some exceptions).
With My Connected Care (MCC), you can:
View lab reports (3 hour delay in MCC)
View diagnostic reports (3 day delay in MCC)
Read clinical notes written by members of your healthcare team
View upcoming appointments and messages from your healthcare team
Complete questionnaires before your appointment
View your allergy information
View pathology reports (10 day delay in MCC)

To enroll in My Connected Care, visit our self-enrollment page My Connected Care - Self-Enrollment. You will need the following information:
First name
Last name
Date of birth, and
1 of the following:
Health card number and medical record number (MRN), OR
Email address (as it appears in our system)
You can find your medical record number (MRN) on documents you were given from the hospital (e.g. a discharge summary, armband, or appointment itinerary)
If you want to enroll in My Connected Care but do not know your MRN and/or do not have an email address listed in your medical record, fill out our Self-Enrollment Assistance Form. A member of our team will review your request and verify your information. We will use the email address you have provided to send you a link to complete the self-enrollment process.

My Connected Care allows you to access your health records from Grand River Hospital and St. Mary's General Hospital. To enroll yourself in my connected care you'll need the following information: your first name and last name as it appears on your health card, your date of birth and either of the following: Your health card number or medical record number at either hospital OR your email address as it's listed in our system. You can find your medical record number on an arm band or documents given to you at the hospital. If you don't have this information but would like to enroll, fill in our self-enrollment assistance form on our website. A member of our team will verify your information and will send you an email with instructions within a few business days.
If you have the information you need. Click on the self-enrollment link on our website. The first step is to fill in the self-enrollment form. Enter your first name and last name as they appear on your health card. Then enter your date of birth. Drop down to select the month and entering four digits for the year. To enroll with personal identifiers, enter your health card number with no letters, dashes or spaces. Then enter the last four digits of your medical record number. Then select, I'm not a robot and complete the puzzle. Confirm your identity and agree to our terms of use. To enroll with an email address already listed at the hospital enter your email address and complete the puzzle. Keep the window open. You will receive a verification code that is valid for 30 minutes. Enter the code and press continue. If you don't receive this email check your spam or junk folder. The last step is to set up your account. First, enter your email address. then enter a strong password using at least three of the criteria. Enter your name and agree again to the terms of use and age restriction. Once you set up your my connected care account you'll be able to see sections of your health record, read messages from your care team, and see upcoming appointments. Here are some final tips to keep your health information safe and secure. Use a unique password you can remember, don't share it. Don't let your browser save your password for you. If you want someone else to have access to your health information, arrange for them to be an authorized representative. If you print or download documents use your own device and keep them in a safe and secure place. Sign out of your account every time. Let's stay connected through My Connected Care.
