Chronic kidney disease (CKD) is a serious, lifelong condition. People with advanced CKD often require complex and intensive care from a multidisciplinary team of healthcare professionals. The Ontario Renal Network (ORN) is the provincial resource for the kidney care system, providing overall leadership and direction to effectively organize and manage the delivery of CKD services in Ontario.
Recently, the ORN introduced the Ontario Renal Plan II (2015-2019), the provincial roadmap to guide how we will all work together over the next four years to continue to improve the lives of those living with CKD in Waterloo Region.
Working together with our partners across Waterloo Wellington region, we will use this plan to increase opportunities to detect and slow the progression of CKD and improve the quality of life and kidney care for current and future patients.

The KidneyWise Clinical Toolkit (opens in new window) provides guidance to primary care providers on which patients are at high risk of developing chronic kidney disease (CKD) and recommendations on how to properly diagnose and best manage the patient to reduce the risk for further disease progression.
