EMG/evoked potential tests are performed at Grand River Hospital, on the 5th floor of the Forty Green (Kaufman) Building. It is located at Grand River Hospital’s KW Campus at 835 King Street West in Kitchener.
A wheelchair-accessible entrance is available at 40 Green Street, on the east side of the hospital property. You are welcome to park in the Green Street parking lot or the main KW Campus parking garage.
When you enter the Forty Green (Kaufman) Building, you will be on the 4th floor of the building. Please go to the 5th floor. Stairs are located at intervals along the main hallway and an elevator is available across from human resources.
How should I prepare for this test?
Please arrive for your appointment at least 15 minutes before your scheduled appointment to allow time to park and have our receptionist register you. Please bring your health card with you and/or your WSIB claim number.
- Wear loose-fitting clothing.
- Please make sure you have no lotions, creams or oils on your skin.
- You’re welcome to wear your eyeglasses/contact lenses.
A physician must order all tests. If your doctor has given you a requisition, please bring it with you. Please bring a current list of medications you are taking.
How long will the test take?
An EMG will take about 30 to 40 minutes. An EMG and consult will take about one hour.
Evoked potential – each part of the exam takes about 30 to 40 minutes. You must have clean, dry hair (no grease), oil, mousse, or gel.
Referring physicians
You may call or fax your request into the special testing unit. If you prefer to fax the requisition, please include:
- Patient’s name;
- Date of birth;
- Ordering doctor’s name and those who need a copy;
- The specific test;
- Pertinent clinical information;
- Reason for the test; and
- Health card number.
