Hours of operation
The adult diabetes education clinic is open 8 am to 4 pm Monday to Friday. The gestational diabetes mellitus clinic is open Wednesdays from 8 am to 4 pm.
Location: The Forty Green (Kaufman) Building, 40 Green Street
We are located on the fifth floor of the Forty Green (Kaufman) building at the KW Campus of Grand River Hospital. Our street address is 40 Green Street, Kitchener.
Our services include the following:
Insulin pump therapy: We show patients how to use their insulin pumps and adjust their insulin usage. Learning how to manage diabetes and making appropriate decisions can help individuals prevent or delay the long-term complications that can affect heart, eyes, feet, kidneys and the nervous system.
Education programs: In our individual and group sessions, we provide education on healthy food choices, healthy weight, regular exercise, diabetes medication, monitoring of blood sugars, sick day guidelines and strategies to achieve and maintain good blood sugar control.
Insulin initiation: We offer information on insulin administration and adjustment for those starting on insulin and those needing adjustments to current insulin routines.
Your health care team includes: registered nurse, clinical dietitian, clinical secretary, and endocrinologist for the GDM

What to bring with you:
- Your Ontario health card;
- Your log book and glucose meter. You can download a log book template that you can print in either Adobe PDF or Microsoft Word formats;
- A list of your current medications;
- Your insulin/insulin pens if using; and
- A list of questions you have.
We encourage you to bring a family member or support person with you to your appointment.
