What is Mount Hope Day Program
The Mount Hope Day Program is a time limited outpatient group therapy program for adults who are able and willing to participate in daily group programming, either in-person or online using the Ontario Telemedicine Network (OTN). Our program focuses on recovery and psycho-educational group content that allows eligible participants to engage in mental health supports while being in the community. Topics discussed may include the following:
- Depression & Anxiety
- Holistic Approach
- Leisure & Wellness
- Distress Tolerance
- Self-Compassion & Mindfulness
- Grief & Loss
- Emotion Regulation
Who can be referred?
- Individuals who are 18 years or older that are either transitioning back to the community after being in hospital, or who are experiencing mental health issues that are concerning enough to consider admission to hospital.
- Referrals are only accepted from Emergency Crisis Services and/or from primary care if the referral does not originate from the inpatient unit.
- Are able to actively participate in daily group programming for a period of 6 consecutive weeks
- Available to attend during day program hours, either for a morning or afternoon 2-hour group session
- Have access to a smart phone and/or the internet if participating in the online group format
What happens when I am a client of the Mount Hope Day Program?
All referred individuals will receive an intake appointment, either in-person or phone, which will then determine appropriateness for group participation. Once confirmed, participants will then be informed of the next available group session start date.
We have a multidisciplinary care team of nursing, social work, peer navigation, and psychiatry.
Psychiatry support is determined at the time of intake, specifically who will monitor, prescribe, and provide psychiatric follow-up.
