What is Mount Hope Day Program

The Mount Hope Day Program is an intense, time limited (7-week) outpatient group therapy program for adults 18 years of age and older with a serious mental illness.

The participant must be able, willing, and agreeable to participate in both group programing and individual sessions dedicated to developing new coping strategies and skills for the improvement of self-management care.

The Mount Hope Day Program is not a DBT or CBT program, however, does draw upon skills such as, mindfulness, distress tolerance, interpersonal effectiveness, communication, and emotion management for the purpose of improving self-management care.

Focus of the program will also be on recovery and psycho-educational support through crisis and safety planning, wellness goal setting, and facilitating connection to community resources.

Participants will be expected to attend two half days (two hours each) of core programming per week along with an hour weekly session with an assigned clinician.

Who can be referred?

  • Individuals who have recently been admitted to the Adult Inpatient Psychiatry Unit and have been referred as part of their discharge plan.
  • Individuals who have presented to the emergency department who have required an emergency crisis assessment who meet criteria for referral.
  • Individuals who have been referred by their family physician for urgent adult outpatient psychiatry services

What is required for successful completion of the program?

  • Are able to actively participate in two half days (two hours each, AM or PM) of group programming along with an hour weekly session with an assigned clinician
  • Have access to a smart phone and/or the internet if participating in an online group format (*if offered)
  • Punctual Attendance (participants may be discharged from the program if missing more than three sessions, group or individual, and if there is frequent disruption to the group due to being late).

What happens once I have been referred?

  • An in-person intake will occur at our office, 850 King St. West to confirm acceptance into the program
  • If accepted, you will be placed on the wait-list for the next available group to begin and will be required to attend an individual orientation session prior to the start of group to complete and/or update a safety plan, review/sign the participation contract, and identify individual goals.
  • Primary support to be provided by one of our multidisciplinary team members (nursing, social work, peer support)
  • Psychiatry support is limited, but available if deemed necessary at the time of referral/intake, if primary care is not available and/or unable to support
