Beginning in February 2025, clerical staff in the Medical Imaging Department will be asking patients if they are willing to answer questions about their personal demographics. Patients can complete the voluntary survey upon registration in Diagnostic Imaging. Patient responses are confidential and filed in their electronic health record.

Here are some frequently asked questions related to this survey.

Why are the Hospitals collecting patient demographic data?

  • In partnership with Ontario Health, this survey is used to help us learn more about the communities we serve
  • Fostering safe, welcoming and inclusive spaces for ALL is a priority at our Hospitals

If I complete the survey questions, where will the data be stored?

  • This information will become part of your electronic health record and be available to all members on your care team
  • The information will also be available to your care team if you have additional clinic visits or are admitted to the hospital again
  • The care team may use this information to inform your care
  • The information will be used by the hospital to help our programs to better serve patients, including you
  • The information will not be shared outside of the hospital at this time

Do staff have training on how to ask these questions in a sensitive manner?

  • All staff have access to electronic learning modules on topics that include:
    • The Importance of Equity, Diversity, Inclusion & Allyship in Healthcare
    • Introduction to Anti-Black Racism
    • Inclusion and Belonging for 2SLGBTQIA+ Individuals in Healthcare and Beyond
    • Caring for Indigenous Older Adults in Waterloo Wellington
  • Clerical staff working in the Medical Imaging department are also receiving in-person training related to this patient centered initiative

How often will you ask me these questions?

  • Questions will be asked every two years to keep the information accurate and up to date
  • If you choose not to participate you can indicate that you do not want to be asked the questions ever again, or that you may be willing to participate at your next visit
