Please make sure you bring the following items with you before an emergency department visit:
- A valid health card;
- The name and address of your family physician. This helps us follow up with your doctor after your emergency stay so they can help you with your on-going care;
- An up-to-date medication list and your medications in their original bottles (including all inhalers, insulin, eye drops, herbal products, vitamins etc.);
- Any information about current health condition/illness that you’re dealing with. Please bring referral notes, test results, or current treatment plans if you have them;
- A list of your allergies; and
- Your insurance information and coverage.
- A small amount of money/change for the payphone, or to buy food or drinks if needed. You should avoid eating until a nurse tells you it’s safe to do so;
- One support person or two parents while waiting or being treated (all children under the age of 16 should be with a parent or guardian);
- A quiet activity for while you wait (i.e. reading material); and
- If you’re expecting an extended stay at the hospital, please bring personal items such as toiletries.
