The Secondary Stroke Prevention Clinic helps people who have had a stroke or are at a high risk of having a stroke by connecting them with testing and services to help prevent them from having a stroke in the future.
It is important to note that the clinic is not a replacement for emergency care. If your stroke symptoms get worse before your appointment, call 911 right away to be taken to the nearest emergency department.
The Secondary Stroke Prevention Clinic is located at Grand River Hospital’s Kitchener-Waterloo Campus (KW Campus) and provides the following services:
- rapid access to diagnostic tests and stroke specialists (internists and neurologists)
- risk factor screening and modification
- referrals to outpatient services as needed
Booking your appointment
The Secondary Stroke Prevention Clinic will contact you to book your initial visit by phone. Depending on the services you require, you many have the option of either an in-person or virtual appointment. Every effort will be made to accommodate your specific needs while providing the most efficient access to the clinic.

Referrals
If you think you would benefit from our services, please ask your doctor if a referral would be right for you. Patients must be referred by either a medical doctor or nurse practitioner to access the clinic’s services. To submit a referral, please download and have your family doctor complete the referral form.
Visiting the clinic
Before your appointment, please complete any testing you’ve been asked to do by your healthcare team. On the day of your appointment, eat your regular meals and take all of your regular medications. Please also bring the following with you to your appointment:
- health card
- a list of all current medications, including prescription medications, over-the-counter medications, vitamins, minerals, and herbal supplements
- a list of past surgeries and significant illnesses, including the year they happened
- any questions you have for your healthcare team
- a family member or support person to attend the appointment with you if desired
During your visit to the clinic, you will meet with a doctor and a registered nurse who specialize in stroke prevention and care. They will discuss your medical history and ask you about your lifestyle to identify any stroke risk factors. They will also provide you with information about how you can lower your risk of having a stroke and may suggest lifestyle changes, order further medical tests, or prescribe you medication.
Directions
The Secondary Stroke Prevention Clinic is located at the KW Campus on the fifth floor of the Forty Green Building. To get to the clinic, enter the Hospital through the Forty Green Building, located at 40 Green Street. Once inside, follow the entranceway straight to the end, and then take a left and walk to the end of the hallway. The elevator will be on your left-hand side. Take the elevator to the fifth floor and follow the signs to the Stroke Prevention Clinic. Please register with the clerk upon arrival.
Parking
A dedicated short-term accessible parking lot is available right outside the Forty Green Building at 40 Green Street (located on Green Street between King and Park Streets). Patients may park in this lot or the KW Campus parking garage.
